Home Care
The Cass County Council on Aging Home Care department mission is to maintain and improve the quality of life for mature adults (over 60 years of age); by providing 24 hour a day basic in-home care, seven days a week (as staff are available). The staff of bonded and trained Care Givers (Direct Care Workers) cover the entire Cass County providing services to individuals and families on both a short and long term basis. |

Services:
Case Coordination
Helps seniors coordinate their care to keep them in the home. |

In-Home Assessment
Determines the individual client needs and develops a care plan to meet their needs. There is no cost for this service. When conducting an in-home assessment we ask that the clients have available to us:
- Medicines they are taking
- Pharmacy information
- Insurance cards
- Medicare card
- List of family or emergency contacts and doctors
- Physical or medical history, including major operations they may have had over their life time
Financial information such as monthly social security, CD’s or any type of income coming into the home monthly, for both or anyone who is to receive services from the COA.

In-Home Home Health
Provides general house cleaning chores, cooking or cleaning, running errands and assisting with personal care. |

Personal Care
Provides assistance with bathing, dressing, grooming, transferring, eating and ambulating. Personal care does not include health-orientated services. |

In-Home Respite
Provides supervised companionship and/or assistance with activities of daily living for individuals in the absence of the primary caregiver(s). These services may include assistance with toileting, eating, ambulating, routine exercise, meal preparation, socialization and personal care. |

Brief Services
Include sharing and providing information; providing an individual with a private hire list, nursing home or senior housing list. |

Information & Referral
If you are looking for additional care beyond what is offered by the COA, we can refer you to skilled care organizations, mental health or other care organizations. (Visiting Nurses, Woodlands Community Health) |

File of Life
Medical information in a bright red pouch which has a magnet on the back and can be placed on your refrigerator. In case an emergency responder is called to your home, emergency information will be available. |

MiCafe
Helps Cass County resident aged 60 or better apply for food benefits from the Food Assistance Program (FAP). No questions are asked about your assets: property, car, home, savings, stock, insurance, etc. MiCAFE helps pay for your food so you can use your income to cover other high living costs! |

RxC.O.P.E. (Community Options for Prescription Enhancement)
Helps seniors over 60 find a prescription drug assistance program by evaluating their eligibility for assistance programs through a computerize drug program. |

Medicare/Medicaid Assistance Program, MMAP
Helps Medicare/Medicaid beneficiaries and their caregivers find their way through the health benefits maze. This free counseling service also includes assistance with Medicare Part D prescription programs. |

Medical Transportation
Provides transportation to out of county medical appointments. The team of volunteer drivers may drive their own cars or drive the handicap accessible van. Transportation is provided to non-Medicaid seniors over 60. |

Meals on Wheels
Delivers hot meals Monday through Friday and or frozen meals one time a week on Tuesday. Meals are delivered to seniors over 60 who live in Cass County. Meals are prepared in the COA kitchen under the dietetic guidelines of an 1800 calorie per day diet. |
